OBJECTIVE: To review our experience with MCA aneurysms managed with microsurgery as

the treatment of first choice.

METHODS: Five hundred forty-three patients with 631 MCA aneurysms were managed with a “”clip first”" policy, with 115 patients (21.2%) referred from the Neurointerventional Radiology service and none referred from the Neurosurgical service for endovascular management.

RESULTS: Two hundred eighty-two patients (51.9%) had ruptured aneurysms and 261 (48.1%) had unruptured aneurysms. MCA aneurysms were treated with clipping (88.6%), thrombectomy/clip reconstruction (6.2%), and bypass/aneurysm occlusion (3.3%). Complete aneurysm obliteration was achieved with 620 MCA aneurysms (98.3%); 89.7% of patients were

To develop an electrophysiological method for intraoperative localization of the trigeminal nerve branches.

METHODS: A series of 55 patients under general anesthesia during radiofrequency thermorhizotomy were studied. The trigeminal nerve root was stimulated through the foramen ovale with the RF electrode. Antidromic responses were recorded from the 3 divisions of the trigeminal nerve in the face. Effectiveness rate, pain relief, Tanespimycin manufacturer recurrence, complications, and patient comfort after the procedure were analyzed.

RESULTS: Reproducible and easily obtained antidromic responses were clearly recorded in every subdivision of the trigeminal nerve in all patients. low frequency-less than 1 case per 3 months, medium-between 1 in 3 months and 1 per month, high-between 1 per month and 1 per week, and very high-greater than 1 per week.

Results: In sampled physicians the mean +/- SD rate of inappropriate prostate specific antigen test use was 30.8% +/- 36.6%. Multiple regression analysis showed that after adjusting for other factors physicians who ordered fewer prostate specific antigen tests (those in the low and medium frequency groups) had a higher rate of inappropriate PSA test use than their counterparts who ordered prostate specific antigen tests with very high frequency (each p < 0.001) Furthermore, physicians in the age groups 30 years or younger and 31 to 35 years had higher rates of inappropriate prostate specific antigen testing than their counterparts in the 41 to 45-year-old group (p = 0.019 and 0.010, respectively).

Conclusions: The likelihood of inappropriate prostate specific antigen screening was significantly and negatively associated with physician clinical experience.
