To determine the practicality and safety of utilizing neuromuscular stimulation (NMES) as a supplementary method for rehabilitation following vascular and general surgery.
A prospective, single-center, single-blind, randomized controlled trial involving parallel groups. This UK secondary care study, a single-centre investigation, will take place at a National Healthcare Service Hospital. Vascular and general surgical patients, 18 years or older, with a Rockwood Frailty Score of 3 or greater when they enter the hospital. The inability or unwillingness to participate in a trial, along with implanted electrical devices, pregnancy, and acute deep vein thrombosis, constitute exclusion criteria. One hundred is the anticipated number of recruits. Participants will be randomly sorted into two groups, active NMES (Group A) and placebo NMES (Group B), ahead of the surgical process. Blinded participants will use the NMES device, one to six sessions a day (30 minutes per session), following surgery, in addition to standard NHS rehabilitation services, until their release. NMES acceptability and safety are assessed by evaluating patient satisfaction with the device, recorded on discharge questionnaires, and any adverse events during the hospital stay. Secondary outcomes of postoperative recovery and cost-effectiveness, determined via diverse activity tests, mobility and independence measures, and questionnaires, are compared between two groups.

By leveraging a multi-component, theory-based approach, the EDDIE+ program works to improve the skills and decision-making ability of nursing and personal care staff in detecting and managing the early signs of deterioration in aged care residents. Twelve RAC-affiliated homes within Queensland, Australia, are collaborators in this research project. A mixed-methods process evaluation, structured by the i-PARIHS framework, will examine the intervention's fidelity, the contextual constraints and opportunities for success, the mechanisms of action driving the program's impact, and the program's acceptability from various stakeholder perspectives. The collection of quantitative data will be prospective, drawing on project documentation for baseline contextual mapping of participating sites, documented activity, and regularly scheduled check-in communications. Using semi-structured interviews with a spectrum of stakeholder groups, qualitative data will be obtained after the intervention. Despite the proven potential of iron and folic acid (IFA) supplements to effectively address anemia in pregnant women, their uptake in Nepal is disappointingly low. Our research proposed that during the COVID-19 pandemic, increasing access to mid-pregnancy virtual counseling twice would contribute to better compliance with IFA tablets compared to receiving only antenatal care.
This non-blinded, individually randomized controlled trial in the plains of Nepal is evaluating two treatment groups: (1) standard antenatal care; and (2) standard antenatal care supplemented by virtual antenatal counseling. For enrollment purposes, pregnant women who are married, within the age range of 13 to 49, who are capable of responding to questions, whose pregnancy is between 12 and 28 weeks, and who plan to live in Nepal for the next 5 weeks are eligible. Two virtual counseling sessions, conducted by auxiliary nurse-midwives, at least two weeks apart, are part of the intervention's strategy for mid-pregnancy. The transition of frail elderly patients from the emergency department (ED) to home environments presents a multitude of interconnected physical and social difficulties. see more To overcome these obstacles, paramedic supportive discharge services utilize in-home assessments and/or interventions. Our intent is to describe current paramedic programs developed to aid in the discharge of patients from the emergency department or hospital, thus reducing the occurrence of unnecessary hospital readmissions. A comprehensive review of the literature regarding paramedic supportive discharge services will depict (1) the importance of these programs, (2) their beneficiaries, referral channels, and delivery teams, and (3) the diagnostic tools and treatment approaches used.
We plan to feature studies which delve into the expanded role of paramedics, focusing specifically on community paramedicine, and the extended post-discharge care offered by the ED or hospital system. Every study design, irrespective of its linguistic origin, will be included without exception. From January 2000 to June 2022, we will incorporate peer-reviewed articles, preprints, and a focused search of the grey literature.
